Grace Pointe Senior Care Community has been providing assisted living, independent living, long-term care, memory care, and senior rehabilitation services to Greeley, Colorado, since 2009.
Grace Pointe is privately and locally owned and staffed by compassionate and skilled caregivers and we are a proud, family-owned business. We helped individuals with all aspects of care.

Our beautifully landscaped senior living campus is located on the western side of Greeley with easy access from US Highway 34 and Interstate 25, and just 2.5 miles from UCHealth Greeley Hospital and 5 miles from Banner Health’s North Colorado Medical Center.

Continuing Care, or seamless care, means that residents and their loved ones can consider Grace Pointe a home through various stages of life. We offer four neighborhoods to accommodate those in Independent Living, Assisted Living, Long Term Nursing Care, and Memory Care.
We also have a post-hospitalization Senior Rehabilitation Center for residents and non-residents, offering in-patient and out-patient therapies. Hospice services may be available to assist residents with pain management and comfort of the mind, body, and spirit.
