Grace Pointe & Medical Partners

Grace Pointe partners with UC Health and Northern Colorado Hospitalists Group (NCH Group) to provide physician services for our campus, including independent, assisted living, memory care, rehabilitation, and skilled nursing.

The NCH Group provides on-site services for all rehabilitation residents at Grace Pointe.  When a rehabilitation patient arrives at Grace Pointe, a dedicated member of the physician team will follow their care to provide consistent, coordinated, and person-centered care.  Once the patient is discharged from Grace Pointe Rehabilitation, their Primary Physician is contacted to resume their care in the community.

Northern Colorado Hospitalists Group provides Physician and Nurse Practitioner services for the residents of Grace Pointe, should a resident choose to use the onsite services. Residents who choose NCH Group’s services benefit from being seen in the privacy of their apartment rather than at a medical clinic. NCH Group also uses Epic as the patient charting system. Epic is a fully integrated, CMS-certified electronic medical record (EMR) system and is the most widely used EMR in the US. To date, more than 50 percent of the US population has their medical records in Epic. More than 1,800 physicians throughout Colorado, Wyoming and Nebraska utilize UCHealth’s shared ambulatory patient record in Epic.

Our History

Northern Colorado Hospitalists Group was founded in 2004 by 3 Hospitalists that joined forces to provide inpatient care at Poudre Valley Hospital. Since that time, the group has grown to over 60 physicians with two additional UC Health hospitals, Medical Center of the Rockies in 2007 and Greeley Hospital in 2019. Two of our founding partners are still with the group, 18 years later.

The Grace Pointe NCH Team

Amber McCollum, MD

Amber McCollum, MD

Dr. Amber McCollum grew up in the Marshall Islands and attended medical school at the University of Hawaii, graduating in 2006. She trained in emergency medicine and worked in various ERs until 2017, when she transitioned to primary care and hospitalist work. In 2021, she found her calling in geriatrics, a field she loves, especially working with the population that has contributed so much to Northern Colorado. Dr. McCollum’s husband is a bison wildlife biologist at CSU. They have two girls and two dogs, and the whole family enjoys spending time outdoors.

Sarah Burton, NP

Sarah Burton, NP

Sarah graduated from the University of Northern Colorado in 2020 with an NP degree and a focus in geriatric and acute care. Prior to graduation, she had been a registered nurse for over a decade. Sarah’s experience as a nurse shaped her passion for caring for the geriatric population and since obtaining her NP, she has served the northern Colorado community in the long term and post-acute rehab setting. Outside of work, she loves spending time with her husband and their many rescue fur-babies. Sarah loves to hike, travel, tend to her garden and flowers, paint and create things, sports, and spending time with her extended family and friends.
