Grace Pointe & Medical Partners

Grace Pointe partners with UC Health and Summit Medical Consultants to provide physician services for our campus to include independent, assisted living, memory care assisted living, rehabilitation and skilled nursing.

The Summit Medical Consultant team provide onsite services to all rehabilitation residents coming to Grace Pointe.  When a rehabilitation patient arrives at Grace Pointe a dedicated member of the physician team will follow their care to provide consistent, coordinated and person-centered care.  Once the patient is discharged from Grace Pointe Rehabilitation their Primary Physician is contacted to resume their care in the community.

Summit Medical Consultants provides Physician and Nurse Practitioner services for the residents of Grace Pointe should a resident chose to use the onsite services.  Residents who chose the services of Summit Medical Consultants have the benefit of being seen in the privacy of their apartment as opposed to a medical clinic.
