Grace Pointe & Medical Partners

Grace Pointe partners with UC Health and Summit Medical Consultants to provide physician services for our campus to include independent, assisted living, memory care assisted living, rehabilitation and skilled nursing.

UC Health Physician, Dr. Jacobs serves as the Director of Rehab at Grace Pointe.  He and the  Summit Medical Consultants team provide onsite services to all rehabilitation residents coming to Grace Pointe.  When a rehabilitation patient arrives at Grace Pointe a dedicated member of the physician team will follow their care to provide consistent, coordinated and person-centered care.  Once the patient is discharged from Grace Pointe Rehabilitation their Primary Physician is contacted to resume their care in the community.

Summit Medical Consultants provides Physician and Nurse Practitioner services for the residents of Grace Pointe should a resident chose to use the onsite services.  Residents who chose the services of Summit Medical Consultants have the benefit of being seen in the privacy of their apartment as opposed to a medical clinic.

Dr. Joseph Jacobs

Dr. Joseph Jacobs

Joseph Jacobs, MD, is the Medical Director and Physiatrist for Grace Pointe’s Rehabilitation unit. He is also affiliated with the UC health systems and the appointed physician for their Grace Pointe residents. He has specialties in Physical Medicine and Rehabilitation, Sports Medicine, and Physical Therapy. He attended medical school at Christian Medical College Ludhiana and graduated in 1971. He completed his residency at Temple University. He has over 50 years of experience in health care.

His wife is also a physician in the Northern Colorado area. They are very busy but enjoy spending time with their family. When they have free time, they also enjoy being outside in the fresh air especially light skiing in the Colorado mountains.

Dr. Rajendra Kadari

Dr. Rajendra Kadari

Rajendra Kadari, MD, recognizing the need for quality post-acute care during his tenure as Chief Hospitalist, established Summit Medical Consultants in 2015. Summit Medical Consultants has quickly grown to a 32-provider practice caring for patients in the Skilled Nursing and Long Term Care environment across the Denver metro area. He completed his Residency Program at East Tennessee State University in 2010 and brings with him extensive experience caring for acutely ill patients as Chief Internal Medicine Physician at OrthoColorado Hospital. Dr. Kadari and his wife are the proud parents of two rambunctious little boys. He enjoys playing cricket, biking, hiking, watching movies, and spending time with his family in the beautiful Colorado outdoors.

Diego Freitas, MD, was born in Buenos Aires, Argentina, and immigrated to the United States with his family when he was eight years old. He graduated college in 1981 and then attended and graduated from Uniformed Services University Of The Health Sciences in 1985, having over 36 years of diverse experience, especially in Internal Medicine.

He spent 20 years serving in the airforce and is a veteran of the 1st Persian Gulf war. He has worked with Operation Renewed Hope in Cambodia, Mexico, Nicaragua, Guatemala, and Japan. He has been married for 47 years, has 3 kids and 12 grandkids. When he is not working, he loves to spend time outdoors with his family enjoying the Colorado lifestyle!

Alexandra Samas, DO

Alexandra Samas, DO

Alexandra Samas was born and raised in Ft. Collins, CO. Dr. Samas received her Bachelor of Science at the University of New Hampshire, where she was also a Division I Soccer player. She completed medical school at NYIT College of Osteopathic Medicine, and completed her residency in Physical Medicine and Rehabilitation at Stony Brook University in Long Island, NY. After 10 years of living in New York, Dr. Samas finally returned to Colorado to be close to family and settle in the state that was always home. She loves being a physician because it allows her to connect with people from all aspects of life, and use her training to improve the quality and function of her patient’s lives.­­When Dr. Samas isn’t at work, she loves spending time with her husband and two kids, cooking, traveling, snowboarding, and riding her Peloton bike.
Leighann Brock, AGACNP-BC

Leighann Brock, AGACNP-BC

After completing her BSN at Colorado State University in Pueblo, CO, Leighann Brock worked at the bedside in critical care for nine years. In 2020, Leighann was awarded her Masters in Adult Gerontology Acute Care Practitioner from the University of Northern Colorado. She is currently pursuing her Doctorate of Nursing degree.

Leighann is a Colorado native. She grew up in Arvada and currently resides in Northern Colorado with her college sweetheart, their daughter, and dog. When she isn’t working, Leighann can be found on the sidelines cheering for her daughter in one of her many sports or enjoying a Colorado Avalanche hockey game.
