When a doctor says “dementia” or “Alzheimer’s,” it can feel like the person you love disappears behind a label. Families often tell us the same thing: “Mom is still Mom. She just needs a little more help.”

That is exactly how we see it at Grace Pointe Senior Care Community in Greeley, Colorado. A diagnosis explains what is happening in the brain. It does not tell you who someone is, what they love, or how they want to spend their day.

What Is Memory Care?

Memory care is specialized senior living for people with Alzheimer’s disease, dementia, or other memory loss. It offers a safe environment, trained caregivers, and daily routines designed around cognitive changes.

The need is growing. The Alzheimer’s Association estimates that more than 7 million Americans age 65 and older are living with Alzheimer’s disease, and many more families are affected by other forms of dementia.

Good memory care does more than keep someone safe. It helps them keep a sense of identity, comfort, and purpose.

Why “Person-Centered” Matters

Person-centered care starts with a simple question: Who is this person?

Not “What stage are they in?” or “What can they no longer do?” Instead:

  • What did they do for a living?
  • What music makes them smile?
  • Are they a morning person or a night owl?
  • What foods, hobbies, and routines feel like home?

The answers shape the care, not the other way around.

How Care Is Built Around Each Resident

Step 1: A Conversation, Not a Checklist

At Grace Pointe, residents work with a care coordinator to create a customized, personal care plan. Family members are part of that conversation. You know your loved one’s history better than anyone, and that knowledge is valuable.

Step 2: A Plan That Fits Today

Every resident’s needs are different. Some need close supervision. Others only need a helping hand with things like walking to meals. Care is matched to what each person needs right now, not a one-size-fits-all package.

Step 3: A Plan That Changes as They Do

Memory conditions change over time. Care plans should too. Because Grace Pointe offers a full continuum of care on one campus, residents can move to a different level of support without leaving a community they already know. That includes:

  • Independent living
  • Assisted living
  • Memory care
  • Long-term senior care
  • Senior rehabilitation

For families, that means fewer stressful moves and less starting over.

Independence Still Matters

There is a common belief that memory care means giving up autonomy. We see it differently.

Grace Pointe is designed to offer as much autonomy as each resident is capable of. That principle guides daily life:

  • Personal space: Residents create their own living space, filled with familiar belongings that help a new place feel like home.
  • Choice: Where safe and appropriate, residents make everyday decisions about their day.
  • Dignity: Support is offered in a way that respects the person, not just the task.

Small choices, like what to wear or where to sit at lunch, help someone feel in control. That matters for confidence and mood.

What a Day Can Look Like

Every day looks different for each resident, but the goal stays the same: a rhythm that feels familiar, calm, and meaningful.

  • Morning: Help getting ready at the resident’s own pace, followed by breakfast
  • Midday: Meals in a shared dining space, with assistance when needed
  • Afternoon: Activities and social time based on interests and abilities
  • Evening: A quiet, comfortable wind-down in their own room

Consistent routines reduce confusion and anxiety for many people living with memory loss. Social connection and gentle activity support well-being, too. You can explore the full range of programs on our Activities page.

Medical Support Close to Home

Memory conditions rarely exist alone. Many seniors also manage heart disease, diabetes, or mobility challenges. Having medical support nearby brings peace of mind.

Grace Pointe partners with UC Health and the Northern Colorado Hospitalists (NCH) Group to provide physician services for the campus. Here is what that means for residents:

  • Physician and Nurse Practitioner services are available on campus for residents who choose them
  • Visits happen in the resident’s own apartment instead of at a medical clinic, which can be much less stressful for someone with memory loss
  • Coordinated records: NCH Group uses Epic, a widely used electronic medical record system, which supports coordinated communication with other providers
  • Continuity in rehab: A dedicated physician team member follows each rehabilitation resident’s care, then contacts the primary physician when the resident is discharged

Residents can also keep their own doctors if they prefer. You can learn more about the team on our Medical Partners page.

Support for Families Too

A memory care diagnosis affects the whole family. It is normal to feel worried, guilty, or unsure. Choosing a community is a big decision, and you should not have to make it alone.

At Grace Pointe, families are partners in care. You can expect:

  • A place to ask questions and get honest answers
  • Guidance on which level of care fits your loved one
  • A team that keeps you informed as needs change

Who creates the care plan?

A care coordinator works with the resident and family to build a customized, personal plan.
