Dr. Masumi Arakane, MD

Dr. Arakane completed her medical education in Ecuador and her Internal Medicine Residency at the University of Iowa Hospitals and Clinics. After working as a hospitalist in Iowa, she brought her expertise to Colorado — and to the residents of Grace Pointe.

Sarah Burton, NP

A University of Northern Colorado graduate with a focus in geriatric and acute care, Sarah Burton has been a registered nurse for over a decade before earning her Nurse Practitioner degree in 2020. Her years of nursing experience shaped a deep passion for the geriatric population, and she has since dedicated her NP career to the long-term and post-acute rehabilitation setting in Northern Colorado.

 Mindy Smith, NP is the Bloom healthcare Provider at Grace Pointe.- Mindy is a Colorado native and loves living in the Centennial State. She completed her undergraduate degree at the University of Northern Colorado and earned a master’s degree from Colorado Technical University. Mindy has almost seven years of nursing experience and is excited to partner with and care for patients and their families in the capacity of a provider. She’s attracted to medicine because it’s a field where the learning never ends and provides opportunities to meet new people and have a significant impact on a patient’s life. She loves working with the geriatric population and is passionate about providing them with excellent care. In her free time, Mindy enjoys reading a good book and being outside with her family, especially tubing on the lake during the summer.

Do I have to use the on-site NCH Group or Bloom physicians? No. Residents who have an existing relationship with a primary care physician can continue that care. The on-site physician services are available for those who choose to use them.
